Support Documents Needed for Application

Information and/or documents you may need when applying for Advance Premium (subsidy) Tax Credits include:

  • Social Security numbers (or document numbers for any legal immigrants who need insurance) for all the individuals included on the application.
  • Birth dates for all the individuals included on the application.
  • Employer income information for everyone in your family (for example, from pay stubs or Forms W-2, Wage and Tax Statements) for all the individuals on the application.
  • Policy numbers for any current health insurance information about any job-related health insurance available to your family.
